Enhanced Service eBaby on Diia Portal
The Diia portal has reactivated the "eBaby" service, enabling users to apply for up to 9 state services with a single application.
In this regard, the Odessa City Council Administrative Services Center has resumed accepting documents for the electronic public service "eBaby".
Parents can submit an application for the "eBaby" service if:
- the child does not yet have a birth certificate;
- the baby was born in a medical institution in Ukraine;
- at least one of the parents is a Ukrainian citizen.
Required documents for application:
- marriage certificate (if available),
- Ukrainian citizen passport of both parents,
- TIN (taxpayer identification number) of both parents,
- UDRN (unique demographic record number) of at least one parent,
- electronic medical birth certificate or birth certificate (form No. 103/o),
- certificate of refusal of the one-time natural assistance package “Baby Package” from the maternity hospital,
- bank social account number of the child assistance recipient (the parent on maternity leave),
- "PrivatBank" card,
- special account for the "Baby Package" (if refusing the natural assistance "Baby Package").
To receive the comprehensive service, parents of newborns must pre-register for an electronic queue.
